Root canals, implants and orthodontics: does an AI calendar choke on them?

It's the most technical objection we get, and also the fairest: in a practice the same treatment takes different lengths depending on the case, some treatments are several sessions separated by weeks or months, and there are waits that can't be shortened. A catalogue-based system, they say, ends up rigid.

Whoever says that has experience. Most digital calendars treat a treatment as a name and a number of minutes, and you can't run a practice on that.

The problem, in three real cases

A root canal on a molar doesn't take the same as one on an incisor, and sometimes it's two sessions. An implant has the surgery, then months of osseointegration, then the crown: three moments separated by a time that depends on the bone, not on the calendar. Orthodontics is a review every four to six weeks for two years.

A «one treatment equals one duration» calendar forces a choice between two evils: book too little and drag a delay all day, or book too much and give away free time. That's why plenty of practices end up running the real calendar in a notebook while the software plays ornament.

How it's solved: durations per case and per dentist

A treatment can have several variants, each with its own duration and its own dentist. When booking you pick the right one and the calendar blocks the correct amount of time. When you don't yet know which it will be at booking, the longest variant is reserved: blocking too much is corrected in a minute when the patient arrives; blocking too little is discovered with two patients in the waiting room.

For multi-session treatments, each session is its own appointment, and the assistant knows the following ones exist: when the time comes, it says so and books the next. Biological waits — an implant's osseointegration — aren't «managed» with a calendar: they're respected, and what the system does is remember for you that this patient has to come back in three months.

The rule behind all of this

The business overrides the software. If you want to put an appointment where the system says it doesn't fit — because you know that patient, because it'll be quick, because it's an emergency — there's a «save anyway» that puts it in and flags it. Nobody asks you anything else.

What the system never does is the opposite: force a time slot on its own account. That asymmetry is deliberate. Software that breaks its own rules only sits two patients on top of each other; software that lets *you* break them only does what you asked.

What we don't cover, and won't pretend we do

Very personal calendar rules: «Tuesdays are surgery only», «this dentist doesn't do two root canals back to back», «I leave half an hour between two extractions». Some are solved with hours and treatments per dentist; others aren't. If your practice lives on a rule that doesn't fit, it shows up in the demo and you'll be told.

Changing a duration mid-treatment: if a session runs long, the appointment is stretched by hand. The calendar doesn't guess.

When something here doesn't fit, the honest alternative is a custom build. That's a legitimate option and we've recommended it to some practices.

What to ask for in any demo

One: ask them to show you one treatment with two different durations. If you have to create two separate treatments with artificial names, the calendar is rigid.

Two: ask them to try putting an appointment where it doesn't fit. If it won't let them, the software overrides you; if it lets them without warning, it protects you from nothing. The right behaviour is to warn and let you decide.

Three: ask them to cancel an appointment and look at the gap. Is it offered on its own to whoever was waiting, or does it sit empty until somebody remembers?
